- Clear copy of PRC ID or Proof of Renewal, if expired
- Copy of latest bank statement (CA/SA) for at least 2 months or list of clients
- Co-maker requirements waived if 10 years in practice and affiliated with various hospitals (subject to CRD approval); dentists & vets exempted
- Basic documentary requirements

Loan Interest Rate (Add-on): 2.5% (for doctors) or 2.75% (for dentists and vets)

Loan Processing Fee:
Metro Manila: 3% + P150
Province: 3% + P200

Loanable Amount: P50K to P250K

Loan Terms: 3 to 9 months
